When submitting your projector lamp for relamping, please include your relamping order form with your shipment. The RelampIt! order form allows RelampIt to gather information about you and your RelampIt! needs. All information gathered is kept confidential.

Please print your RelampIt! order form and provide your signature for relamping authorization, this will also aide in a speedy return of your projector lamp. We happily accept MasterCard, Visa, American Express and Discover, for additional payment arrangements such as payment terms or Purchase Orders, please contact us.

RelampIt! is not responsible for any lamps received without the requested paperwork (i.e. order form or Purchase Order) for more than 14 days. Following 14 days, projector lamps that are unclaimed will be submitted to the RelampIt! Recycling Center. It is essential that ALL customers submit a completed order form or purchase order with their request for service.

When a credit application is submitted, processed and approved for Net 30 terms with RelampIt!, the terms are as follows: All invoices must be paid within 30 days of issuance, Late payment fees will be assessed and added to the invoice. Late payment fees will be assessed at a rate of 5% per month over 30 days.
